The following are additional fees that are NOT covered by insurance and are payable at the time of service.

  • *Appointments must be cancelled at least 24 hours before scheduled time or will be subject to a $50.00 late cancelation fee and/or a $150 no show fee and will be charged to the credit card on file. A valid payment card must be kept on file at all times.
    *No show or late cancelation fees for sessions outside of office hours, including Fridays, will be charged at $250.00.  (Before 9am, after 6pm and anytime on Friday, Saturday, or Sunday)
  • Court/Legal involvement fees start at $500/hr and will be billed in full hour increments.
  • Disability paperwork, FMLA, and records requests for such are $200 for the initial paperwork and $100 for each follow up request. This fee provides for the provider to review your chart, prepare the documents, and provide them to the requesting entity. These forms are not covered by your insurance provider, your disability company, or your employer.

​     ​​​​Counseling services are typically covered by medical insurance, however, personal financial responsibility may vary depending on your insurance coverage. It is your responsibility to verify your benefits and if necessary, receive an authorization for services prior to attending your appointment. It is also your responsibility to keep the provider updated of any changes to coverage. Failure to do so in a timely manner may result in your responsibility of total outstanding charges. This provider is no longer participating in Employee Assistance Program sessions, regardless of non-EAP network status. Per the No Surprises ACT of 2020, you have a right to a transparent estimate of costs for counseling services and a good faith estimate of all out of network or private pay services will be provided upon request.
